The National Cambodian American Diabetes Project (NCADP) is a coalition of five community organizations. Project activities are implemented in five project sites and
are connected using a national videoconferencing network that allows us to effectively function collaboratively as one project.

The project sites represent the resettlement patterns in which Cambodians live: high density (over 7,000), moderate density (1,000-5,000) and low density areas (<1,000). The dissemination and outreach patterns of each project site are different and depict specific access and health challenges in their area.
